Pinoy Dental Card (2625/Annum)

A minimum volume requirement is 500 initial members. DENTAL CARE SERVICES: * Free and Unlimited Consultation and Dental Examinations; * Simple Tooth Extraction, except molars and surgery for impaction (unlimited/maximum of 3 teeth per day); * Twice a year Oral Prophylaxis (mild to moderate) 1st availment upon first payment, 2nd availment after six (6) months from the last availed. * Temporary Fillings, per quarter 1 tooth only; (Maximum of 3 teeth per year) * Adjustment of denture; * Recementation of jacket crown inlays and on lays; * Medication and/or prescription for dental pain; * Treatment of dental related pain excluding cost of prescribed medicines; * Emergency desensitization of hypersensitive teeth; * Annual dental examination (within the dentist / Dental Clinic only); * Orthodontic and Aesthetic and dental consultation; * Lightcure / permanent filling 1st availment (4 surfaces only) upon 1st payment. 2nd availment (4 surfaces again) after six months from the last availed.
